The subject matter of the proposed Settlement Agreement concerns matters for which the Respondent may be disciplined as an Approved Person of the MFDA, pursuant to ss. 20 and 24.1.1 of By-law No. 1 of the MFDA.

PARTICULARS
- The proposed Settlement Agreement concerns the allegations that the Respondent:
- between March 2015 and March 2016:
- cut and paste five (5) client signatures from account forms previously signed by the clients onto a new account form;
- signed the signature of one (1) client on an account form;
- re-used one (1) signature page from an account form that was previously signed by the client;
and submitted the forms for processing, contrary to MFDA Rule 2.1.1; and
- on or about June 25, 2015, obtained, possessed, and used to process a transaction, one (1) pre-signed account form, contrary to MFDA Rule 2.1.1.
- between March 2015 and March 2016:
- MFDA settlement hearings are typically held in the absence of the public pursuant to section 20.5 of MFDA By-law No. 1 and Rule 15.2(2) of the MFDA Rules of Procedure.
